Benefit-Risk Profile of Intravenous Immunoglobulin (IVIG) and Subcutaneous Immunoglobulin (SCIG) in CIDP: the PATH Study

PATH investigated efficacy and safety of SCIG (IgPro20, Hizentra®) as maintenance therapy for CIDP. Before randomization to IgPro20 or placebo, subjects underwent IVIG withdrawal and, upon clinical deterioration, were re-stabilized with IVIG (IgPro10, Privigen®). IgPro10 and IgPro20 are manufactured identically with the only differences being the final immunoglobulin concentration and administration route. The benefit-risk profiles of IgPro10 IVIG and IgPro20 SCIG are evaluated here.

To detail the benefit-risk profiles of IgPro10 IVIG and IgPro20 SCIG in chronic inflammatory demyelinating polyneuropathy (CIDP).

IVIG re-stabilization comprised an initial dose of 2g/kg followed by 3–4 doses of 1g/kg at 3-week intervals. Subjects were randomized to weekly SCIG maintenance therapy (0.2 or 0.4g/kg) or placebo for 24 weeks. The adverse event (AE) profile and relapse (change in adjusted Inflammatory Neuropathy Cause and Treatment [INCAT] score) were analyzed for both products.

207 subjects received 1620 IVIG infusions, 115 subsequently received 4225 SCIG infusions. During IVIG re-stabilization, 48.3% of subjects experienced AEs (rate/infusion 0.18); 27.5% were causally-related. With SCIG, AEs were reported in 58% (rate/infusion 0.08) and 52% (rate/infusion 0.05) of 0.2 and 0.4g/kg subjects, respectively; causally-related AEs were reported in 29.8% and 34.5% of subjects, respectively. The most frequent AEs were headache (IVIG: 16.4%) and local reactions (SCIG: 19.3% 0.2; 29.3% 0.4); the majority (65% and 95%, respectively) were mild. Headache and nausea were more common with IVIG (16.4% and 4.8%) than SCIG (7%/6.9% and 0%/0%). With IVIG, 7 hemolysis AEs occurred (all non-serious and resolved without transfusion); none occurred with SCIG. No thromboembolic events, renal failures or deaths were reported. In total, 83% of subjects re-stabilized on IVIG; subsequently, 81% did not relapse on 0.4g/kg SCIG (0.2g/kg SCIG: 67%; placebo: 44%).

IVIG and SCIG had the expected effectiveness reported in literature, while rates of systemic AEs were lower with both SCIG doses.
