Title The Safety and Efficacy of Daclizumab HYP in Relapsing-Remitting Multiple Sclerosis in the SELECTION Extension Study: Primary Results

Background SELECT was a 1-year, placebo-controlled study of DAC HYP in RRMS.

Objective

Design/Methods 517 patients entered SELECTION (92% of eligible patients). SELECT placebo patients (n=170) were randomized to initiate monthly 150mg or 300mg subcutaneous DAC HYP. SELECT DAC HYP patients (n=347) were randomized to continue their current dose or to a 24-week treatment interruption (washout then original dose reinitiation). Results of the DAC HYP dose groups were pooled.

Results 92% (n=474) of randomized patients completed the 52-week treatment phase. In patients who initiated DAC HYP in SELECTION, the 52-week ARR was reduced by 59% (0.18 vs. 0.43; P<0.001) and the proportion of patients with confirmed 3-month disability progression was reduced by 50% (5% vs. 10%; P=0.033) versus the prior year. Among patients who remained on DAC HYP over 2 years, the ARR achieved in year 1 was sustained in year 2 (0.148 vs. 0.165), and there were fewer new/newly enlarging T2 lesions in year 2 versus year 1 (1.2 vs. 1.85; P=0.032). 88% were free of confirmed disability progression at 2 years. The treatment-interruption group showed no evidence for disease rebound (mean Gd+ lesions at pretreatment baseline vs. end of washout: 1.6 vs. 1.1). One death due to autoimmune hepatitis occurred in the 300mg treatment-interruption group. The incidence of serious infections (2% vs. 2%) and serious cutaneous events (1.1% vs. 1.0%) was similar in SELECTION versus SELECT whereas AST/ALT elevations >5x ULN were less common (1.5% vs. 4%).

Conclusions The efficacy of DAC HYP was sustained through the second year of therapy and the safety profile was similar in years 1 and 2. There was no evidence of rebound disease activity after washout.
