Time Course of Neurological Deficits in Pediatric Arterial Ischemic Stroke

The course of recovery following pediatric arterial ischemic stroke (AIS) remains incompletely understood. Unlike adults, deficits in children may only emerge with maturation, months and years post-stroke. There are few longitudinal studies investigating outcome over time in pediatric stroke.
Data were obtained from the Canadian Pediatric Ischemic Stroke Registry for patients seen at the Hospital for Sick Children with AIS from 1992 to 2011. We studied patients with standardized neurology follow-up (Pediatric Stroke Outcome Measure, PSOM) available for at least three time points. For some visits, PSOM was retrospectively abstracted from clinic dictations.
PSOM scores were analyzed for 201 patients, with a total of 771 visits. The sample comprised 128 (64%) males, with mean age of stroke onset at 6.31 years (range, 0.09-17.60 years), and final PSOM mean 79.2 months (range, 9-189 months) post-stroke. In all domains, neurological deficits improved during the months post-stroke, but new deficits emerged over subsequent years. At final follow-up, Seventy-six percent of patients had some neurological deficit. Stroke onset prior to age two is associated with emergence of deficits in all domains (p=0.014 in the sensorimotor domain, p=0.016 language, and p=0.048 cognitive/behavioural).
Deficits emerge in all domains following pediatric stroke, emphasizing the importance of longitudinal follow-up and rehabilitation. Late emergence of deficits may also be contributing to the variability of recovery reported in previous studies.
