Patterns and Characterization of Acute Prescription Migraine Medication Use: Results From the Chronic Migraine Epidemiology and Outcomes (CaMEO) Study

Appropriate prescribing of APMM is not optimal, with claims-based analyses showing that 15%-43% of those with migraine currently do not receive acute prescription medications.
1) To quantify and characterize use of acute prescription migraine medication (APMM) among respondents meeting criteria for migraine in the United States. 2) To characterize migraine-related burden with respect to APMM use status and reasons for never using APMM.
This analysis of the longitudinal, Web-based Chronic Migraine Epidemiology and Outcomes (CaMEO) Study data compared respondents meeting modified International Classification of Headache Disorders, 2nd Edition migraine criteria. Respondents were categorized by current APMM use with respect to treatment decisions, headache characteristics, and headache-related disability.
13,624 respondents were divided into current users (n=3121 [22.9%]), lapsed users (n=1719 [12.6%]), and never users (n=8784 [64.5%]) of APMM. Current users had the highest mean headache day frequency (7.3 days/month), rates of physician-diagnosed migraine (78.8%), and moderate or severe migraine disability (Migraine Disability Assessment grade III or IV: 59.8%). Moderate or severe disability was lower among lapsed users (41.8%) and never users (28.8%). 46.7% of current users had used barbiturates or opioids to treat headache or had them on hand. Most common reasons never users gave for not using APMM were that their headaches were not that serious (53.2%), satisfactory response to over-the-counter medications (42.6%), or they did not want to take a prescription medication (30.4%).
The vast majority of those with migraine in the general population have never used APMM. As 28.8% of this group has moderate/severe headache disability, many would benefit from these treatments. Among current users, the high rates of migraine-related disability and opioid and barbiturate use reflect additional unmet treatment needs.
