The contribution of white matter lesions to whole brain and deep grey matter atrophy in patients with MS

Whole brain and deep grey matter atrophy occurs at a greater rate in MS patients than healthy controls and correlates with clinical disability more strongly than T2-lesion volume (T2LV). Atrophy may reflect primary neurodegeneration, or a response to lesions in afferent or efferent projections.
To determine the relation of whole brain and deep grey matter atrophy with T2LV in a large clinical care cohort of patients with MS. 
Demographic and disease characteristics, neuroperformance tests, and patient reported outcomes were recorded from MS patients at routine office visits. Clinical brain MRIs obtained +/-90 days were analyzed via a fully automated method for T2LV, whole brain fraction (WBF), and deep grey matter volumes (thalamus, putamen, caudate, and hippocampus). Spearman correlation coefficients were used to assess associations; significance was set at p<0.001. Linear regression models were used to explore predictors of T2LV and thalamic volume after adjusting for age.
In a linear regression model of data from 997 patients, significant associations with T2LV were found for thalamic volume (standardized regression coefficient [β] = -0.31, p<0.0001), WBF (-0.28; p<0.0001), hippocampal volume (-0.11, p=0.0003), and putaminal volume (-0.08, p=0.02) but not caudate volume. WBF was the strongest predictor of thalamic volume (β = 0.62) with smaller contributions from T2LV (-0.30) and age (0.12); all p<0.0001. All measured volumes correlated weakly with Patient Determined Disease Steps, Timed 25 Foot Walk, and 9-Hole Peg Test.
The relation of T2LV and brain/thalamic atrophy suggest that secondary neurodegeneration could be an important contributor to atrophy. As the thalamus has the greatest connectivity of the deep grey matter structures, it may be particularly vulnerable to neurodegeneration and serve as an indicator of global injury.
