Title Association between Parkinson’s disease and Acute or Subacute Subdural Hemorrhage, a Retrospective Study

Objective To determine the association between Parkinson’s disease and Parkinsonism with symptomatic subdural hematoma outcomes.

Background Patients with Parkinson's Disease (PD) and Parkinsonism have higher rates of falls than age-matched controls. The leading cause of subdural hematoma (SDH) is fall, implicated in up to 60% of cases. There have been many case reports describing cases in which patients’ new onset or rapidly deteriorating Parkinsonism was caused or aggravated by their SDH. In patients with SDH, PD has not been systematically examined as a potential factor influencing outcomes. Here, we attempt to investigate the association of PD with SDH.

Design/Methods A multicenter, hospital-based, retrospective, cohort study was designed analyzing data from 160+ hospitals in the HCA Healthcare USA network. 12,042 patients 40 years and older were identified who had been admitted to one of these hospitals between 2016 and 2019 who had received a diagnosis of symptomatic SDH. Of these, 2.75% had a concurrent diagnosis of PD or Parkinsonism. Several endpoints were evaluated to determine outcomes in these patients. Linear regression for continuous outcomes, logistic regression for binary outcomes, and multinomial logistic regression for discharge outcomes were used during analysis.

Results Patients with symptomatic SDH and PD/ Parkinsonism did not have significantly different length of stay or cost of hospitalization. In addition, these patients did not have higher risk of medical complications including infectious, cardiovascular, GI, neurological, renal and respiratory complications. However, Parkinson’s disease is significant (CI= 1.65-2.81) in estimating the likelihood of discharge to long term nursing facility vs home in patients hospitalized with SDH.

Conclusions Although patients with PD or Parkinsonism who experience SDH have similar rates of in hospital length of stay, cost, and medical complications, they are 2.152 times as likely to experience discharge to long term nursing facility over home discharge, compared to patients without Parkinson’s disease.
