Participants should be able to describe ways to fund transition from pediatric to adult care with regards to: 1) Methods for optimizing billing when patients are evaluated by multiple providers within the same clinic (including adult and pediatric providers), 2) Billing and coding strategies to maximize reimbursement for care transition coordination, 3) Guiding patients for funding care, including by leveraging dual Medicare-Medicaid eligibility.
